SPICE itself is extremely difficult to learn and use, so programs such as Multisim provide an intuitive front end for the powerful SPICE engine.
SPICE is the industry standard circuit simulation engine, developed here at Berkeley. Multisim is a circuit simulator powered by SPICE.
